Objective: Symptom improvement is often examined as an indicator of a good outcome of accessing mental health services. However, there is little evidence of whether symptom improvement is associated with other indicators of a good outcome, such as a mutual agreement to end treatment. The aim of this study was to examine whether young people accessing mental health services who meaningfully improved were more likely to mutually agree to end treatment.Methods: Multilevel multinomial regression analysis controlling for age, gender, ethnicity, and (...) referral source was conducted on N = 8,995 episodes of care [Female = 5,469, 61%; meanAge = 13.66 years] using anonymised administrative data from young people's mental health services.Results: Compared to young people with no change in mental health difficulties, those showing positive meaningful changes in mental health difficulties were less likely to have case closure due to non-mutual agreement. Similarly, they were less likely to transfer or end treatment for other reasons than by case closure due to mutual agreement.Conclusion: The findings suggest that young people accessing mental health services whose symptoms meaningfully improve are more likely to mutually agree to end treatment, adding to the evidence that symptom improvement may be appropriate to examine as an indicator of a good outcome of accessing mental health services. Referring to certain lexical units of Roget’s Thesaurus, the authors consider in the article philosophical interpretation of verbal means for creation and further transformation of culturally significant senses and meanings that are aligned with death and dying. Thus, the given study involves one of the most famous and historically significant treasuries of language into plentiful and practically oriented philosophical examination. The authors propose a heuristic scheme to classify lexical units according to their structure and meaning with taking general and peripheral (...) meanings into account. The article hermeneutically reveals linguistic and cultural sources of common images and concepts of death and dying. The authors make up a conclusion about sense-generative, gnoseological, world-view, cultural and communicative functions of the above-mentioned units that point to death and dying. Thus, Roget’s Thesaurus can be considered a helpful guide to philosophical examination of culturally prescribed or determined concepts of death and dying, of verbally expressed attitudes to death issues. The possible role of the neurotransmitter serotonin in human affiliative behavior is under-examined in the review by Depue & Morrone-Strupinsky (D&M-S). This commentary reviews evidence indicating that serotonin not only inhibits aggressive behavior that may be detrimental to affiliative bonds with others in a social group but serotonin also enhances prosocial behaviors that may facilitate ties to the social group.

Study of concept of Brahman in Upanishads with special reference to the philosophy of Śāṇḍilya.

Respecting privacy and patients’ satisfaction are amongst the main indicators of quality of care and one of the basic goals of health services. This study, carried out in 2007, aimed to investigate the extent to which patient privacy is observed and its correlation with patient satisfaction in three emergency departments of Tehran University of Medical Science, Iran. Questionnaire data were collected from a convenience sample of 360 patients admitted to emergency departments and analysed using SPSS software. The results indicated that, (...) according to 50.6% of the patients, the extent to which their privacy was respected was described as either ‘weak’ or ‘average’. Spearman’s coefficient indicated a significant correlation between respecting privacy and the patients’ satisfaction about the various aspects of privacy studied. Considering the levels of privacy observed together with the patients’ degree of satisfaction, it is imperative that clinical professionals address both aspects from conceptual and practical viewpoints. A method is given to determine whether or not the distribution functions describing the two spin measurements in the spin-s Einstein-Podolsky-Rosen experiment are compatible with the existence of distributions describing three spin measurements (not all of which can actually be performed). When applied to the spin-1/2 case the method gives the results of Wigner, or of Clauser, Holt, Horne, and Shimony, depending on whether or not the two-spin distributions are assumed to have the forms given by the quantum theory. Generalizations (...) of the conditions of Wigner or of Clauser et al. to the spin-1 case are explicitly calculated. The spin-3/2 case is examined in some simple geometries to show that an apparently monotonic trend toward local realism as s increases from1/2 to1 is, in fact, violated when s increases from1 to3/2. The analysis is based on a novel representation of the modulus squared of a rotation matrix element. The structure of that matrix element responsible for the restoration of local realism in the classical (large s) limit is identified, but a rigorous treatment of the classical limit is not attempted. The higher-spin results are significantly stronger than those given by Mermin's spin-s Bell inequality. (shrink)

On the life, philosopy, and mythological interpretations of Śukadeva as portrayed in Puranas; a study.

BackgroundThe potential contribution of community engagement to addressing ethical challenges for international biomedical research is well described, but there is relatively little documented experience of community engagement to inform its development in practice. This paper draws on experiences around community engagement and informed consent during a genetic cohort study in Kenya to contribute to understanding the strengths and challenges of community engagement in supporting ethical research practice, focusing on issues of communication, the role of field workers in 'doing ethics' on (...) the ground and the challenges of community consultation.MethodsThe findings are based on action research methods, including analysis of community engagement documentation and the observations of the authors closely involved in their development and implementation. Qualitative and quantitative content analysis has been used for documentation of staff meetings and trainings, a meeting with 24 community leaders, and 40 large public and 70 small community group meetings. Meeting minutes from a purposive sample of six community representative groups have been analysed using a thematic framework approach.ResultsField workers described challenges around misunderstandings about research, perceived pressure for recruitment and challenges in explaining the study. During consultation, leaders expressed support for the study and screening for sickle cell disease. In community meetings, there was a common interpretation of research as medical care. Concerns centred on unfamiliar procedures. After explanations of study procedures to leaders and community members, few questions were asked about export of samples or the archiving of samples for future research.ConclusionsCommunity engagement enabled researchers to take account of staff and community opinions and issues during the study and adapt messages and methods to address emerging ethical challenges. Field workers conducting informed consent faced complex issues and their understanding, attitudes and communication skills were key influences on ethical practice. Community consultation was a challenging concept to put into practice, illustrating the complexity of assessing information needs and levels of deliberation that are appropriate to a given study. Recognition that the Principle of Respect for Autonomy fails to work in family-centric societies such as Singapore has recently led to the promotion of relational autonomy as a suitable framework within which to place healthcare decision making. However, empirical data, relating to patient and family opinions and the practices of healthcare professionals in Confucian-inspired Singapore, demonstrate clear limitations on the ability of a relational autonomy framework to provide the anticipated compromise between prevailing family decision-making norms and adopted Western led atomistic (...) concepts of autonomy. Evidence suggests that despite a growing infusion of Western influence, there is still little to indicate any major shift to individual decision making, particularly in light of the way society and healthcare are structured. Similarly, the lack of employing a shared decision-making model and data that discredit the notion that the complex psychosocial and cultural factors that affect the decision making may be considered “content neutral” not only prevents the application of relational autonomy but questions the viability of the values behind the Principle of Respect for Autonomy. Taking into account local data and drawing upon a wider concept of personhood that extends beyond prevailing family-centric ideals along with the complex interests that are focused upon the preservation of the unique nature of personhood that arises from the Ring Theory of Personhood, we propose and “operationalize” the employing of an authoritative welfare-based approach, within the confines of best interest decision making, to better meet the current care needs within Singapore. In this paper, we defend Socratic studies as a research programme against several recent attacks, including at least one recently published in Polis. Critics have argued that the study of Socrates, based upon evidence mostly or entirely derived from some set of Plato’s dialogues, is sfounded upon faulty and indefensible historical or hermeneutical technique. We begin by identifying what we believe are the foundational principles of Socratic studies, as the field has been pursued in recent years, and we then show (...) how the research programme that derives from accepting these principles is not defeated by any of the most common recent criticisms of it. Specifically, we argue that challenges to sorting Plato’s dialogues by date, more general challenges to historicist interpretations of Plato’s dialogues, as well as recent literary criticisms of Socratic studies all fail to undermine the research programme. We conclude with some thoughts about how and why Socratic studies has proved itself a valuable and fruitful research programme. (shrink)

Study with commentaries of Annambhaṭṭa's Tarkasaṅgraha compendium of the basic concepts and terminology of the Nyāya and Vaiśeṣika philosophy.

The study reports on the results of an empirical investigation of the education and recruitment processes used in HIV vaccine trials conducted in South Africa. Interviews were conducted with 21 key informants involved in HIV vaccine research in South Africa and three focus groups of community advisory board members. Data analysis identified seven major themes on the relationship between education and recruitment: the process of recruitment, the combined dual role of educators and recruiters, conflicts perceived by field staff, pressure to (...) achieve recruitment targets, problems in achieving comprehension, accountability and education as capacity building. The results raise ethical concerns about the adequacy of current informed consent processes in these settings. The study findings bear directly on current debates about issues of exploitation and the scope of moral responsibilities of researchers and funding agencies to assure that HIV clinical prevention research is conducted ethically. The technical literature actively discusses ideas for creating digital twins to solve a wide range of problems that arise for enterprises of various kinds. Advances in the areas of information technology and the development of the necessary software really make it possible to obtain important new results through the creation of digital twins. This paper proposes the concept of building a network of digital twins, used to solve a number of actual problems in complex telecommunication systems. Two applications are considered (...) examples of such tasks. The first application is the organization of information feedback for enterprises involved in the life cycle of a telecommunication network: “elaboration of modernization principles, development of equipment, design, construction, and operation.” The second application is monitoring traffic, including its atypical behavior, in order to avoid the recurrence of erroneous actions in case of emergency situations. Another important feature of the digital twin network is that it will become an effective tool for conducting interdisciplinary research. (shrink)
